MAIN REQUIREMENTS FOR GETTING PIERCED

REQUIREMENTS FOR THE CHILD:

We will need a birth certificate or document listed below and shot records or shot exemption forms. 

As well as parent/legal guardian with their ID.

• Acceptable forms of ID for the parent/ and legal guardian include the following:

1. Passport 
2. Military ID
3. Driver's License
4. Permit etc

*ID must be valid and not expired.

STUDIO POLICY REQUIRES:

• Children 3 months - 10 years old, must be accompanied by a parent/legal guardian and must provide identification and proof of age and relationship to the guardian in order to be pierced.  

• If the parent has a different last name than the minor, then legal documentation (birth certificate, marriage certificate, divorce papers, adoption papers, etc.) must be provided to verify the relationship.

• If the legal guardian is not the parent, then legal documentation (adoption papers, power of attorney giving full guardianship without limitations, etc.) must be provided to verify the guardian’s legal right to sign for the minor.

*legal guardian must be present at ALL times during the piercing

• Anyone under the age of 18 must be accompanied by a parent/legal guardian and must provide identification and proof of age and relationship to the guardian in order to be pierced.

• Acceptable identification and proof of age and relationship include the following:

1. Passport 
2. Military ID
3. Driver's License
4. Permit etc

*ID must be valid and not expired.

• Any of the forms of acceptable identification listed above from both parent and minor on which either last names or addresses match.

• If the parent has a different last name and or address than the minor, then legal documentation (birth certificate, marriage certificate, divorce papers, adoption papers, etc.) must be provided to verify the relationship.

• If the legal guardian is not the parent, then legal documentation (adoption papers, power of attorney giving full guardianship without limitations, etc.) must be provided to verify the guardian’s legal right to sign for the minor.

• A school issued photo ID or yearbook picture accompanied by a birth certificate is acceptable 

*legal guardian must be present at ALL times during the piercing

We will need a government-issued ID. Read below for acceptable forms of ID.

*we do not accept broken, cracked, or expired ID's

• *only one guest will be allowed to accompany the person getting pierced in the booth.
• *no one under the age of 18 may be present in the booth during a private piercing.
